Home Care Support Services
Before you receive the Home Care Service a social worker will assess your needs, and ensure you meet the eligibility criteria. An Occupational Therapist may also be involved in this initial assessment. The social worker will then agree a care plan with you. You will be given a copy of your care plan.
When is the service available?
Our Home Care Service operates each day of the year between the hours of 7am to 10pm each day. The number of visits you are offered will depend on your needs. This may include a number of visits per day, or just one or two per week. The time of visits will be agreed with you to fit your lifestyle and assessed needs.
In addition care workers can be provided to sleep-in overnight or on a waking-night basis as well as providing full 24-hour a day cover.
What services do we provide?
We will support you to stay in your own home by providing personal care and practical support. Your care plan will specify the kind of care and support you will receive but this may include:
Personal care such as
- Washing and bathing
- Toileting
- Dressing/undressing
- Assistance in and out of bed
- Feeding
- Personal hygiene
Practical care such as
- Preparation of meals
- Laundry
- Medication
- Shopping
- Essential cleaning
Housekeeping will not be in your care plan as it is not funded by Social Services. We can however provide this service to you in a private arrangement.
What are the rules relating to the home care service?
Our rules are relatively straightforward. To provide you with the necessary care and support we require you to co-operate. This means that:
- you are available on the days and times agreed in your plan, and that
- you are not abusive or aggressive toward any of our staff.
